A father of seven was not informed by hospital staff that he was facing a terminal illness, as revealed by an investigation from the health ombudsman.

William Chapman, affectionately known as Syd, was admitted to the Countess of Chester Hospital in July 2021 due to shortness of breath. Upon admission, doctors assured him that there was no need for concern and that he would recover.

However, the 58-year-old learned of his diagnosis—pulmonary fibrosis, an irreversible lung condition—only in December of that year, when his general practitioner mentioned it casually during a phone call, a shocking revelation that came eight months prior to his passing.
